3. Authenticated copy of certificate of eligibility/rating/license.
4. Authenticated Copy of Transcript of Records, including graduate/post-graduate, if applicable.
5. Certificate of Employment, Contract of Service, or Duly signed Service Record, if applicable.
6. Original copy of Certificate/s of training, if applicable.
7. Photocopy of the Performance Ratings covering one (1) year performance in the last rating period/s prior to the assessment, if applicable.
8. Omnibus Sworn Statement and Certification/Waiver pursuant to Data Privacy Act.
9. Photocopy of Last Appointment, if applicable.
10. Photocopy of Performance Rating obtained from the relevant work experience, if performance rating in item no. 7 is not relevant to the position to be filled; and
11. Means of Verification (MOVs) showing Outstanding Accomplishments, Application of Education, and Application of Learning and Development reckoned from the date of late issuance of appointment.
